Application Process:
- Download application from below link.
- Fill the application and submit to your grama / ward sachivalayam.
- You should carry Aadhar linked mobile number with you in order to authenticate via OTP.
Waiting List (Receipt Downloading Process):
Step 1: Click on the link then you will be redirected to new page.
Step 2: Now enter SADAREM Id in sadarem column and enter captcha displayed below.
Step 3: Hit on search option then your application will be displayed on screen.
Step 4: You can download from download option on screen or you can take a screenshot or take a print by using command (ctrl+p).
Note:
You can find hospital name, address and time slot given to you at the bottom of the receipt.
SADAREM Slot Booking Process in DA/WEDPS Login:
- Click on the below link then then you will be redirected to VSWS site.
- Click on the login, enter credentials and captcha.
- After successful login click on the Other Services.
- After clicking on other services option you will be redirected to old GSWS site.
- In "Health, Medical, and Family Welfare" option you can find "Family Welfare" option click on it.
- Click on "Slot Booking for Sadarem Certificate".
- Now enter beneficiary name, mobile number, and beneficiary Aadhar number and submit.
- Enter Aadhar number in column of beneficiary Aadhar Number and click on tick box for declaration and click on "Generate OTP".
Note :
If you find sadarem id while applying then applicant has to visit hospital in which he/she before attended for examination and request them to delete old sadarem id. After deletion of old certificate only applicant can able to apply for new sadarem id.
- Enter 6 digit OTP received on your Aadhar registered mobile number and click on "Validate OTP".
- Categories are listed as per hospital facilities, Select your category and click on "Submit".
- Details will be displayed as per Aadhar records.
- Enter beneficiary name in Telugu version in "Name to displayed on card".
- Select martial status (married/unmarried/widow/widower/diverse).
- Enter applicant mole details at identification column.
- Enter Father or spouse name at that particular column.
- Select beneficiary mandal, panchayath, town/village, habitation/ward no, phone number details should be entered correctly.
- Now select which hospital you want from dropdown.
- Click on submit. you will get a pop up.
- Press ok for that pop up then sadarem id will generated and linked to beneficiary Aadhar number simultaneously.
